Wealth") is remunerated for its services as
insurance broker by the receipt of commission paid by
insurers. Your agreement to proceed with this insurance
transaction shall constitute your consent for LOGOS Wealth
to receive commission and also to receive your insurance
application and policy related information from insurers. |

居所內所有建築物及內裡結構均屬合法建造，並沒有任何違法加或改建。 You
confirmed that you as the Applicant must be the legal
owner or occupier of the insured home. And the
insured home is a private building, house, apartment or
flat situated in the HKSAR and occupied solely for
residential purpose. The Insured home is also constructed
with bricks, stone or concrete and roofed with concrete
and the age of your home building is within over 50 years.
All structures of the home is legally constructed without
any illegal alternations. |

閣下確認該投保的居所不會空置超過連續30天以上。而且閣下已經及將會採取所有合理謹慎措施，以維持閣下該投保的居所、所有其內家居財物及任何使用於閣下投保的居所的物品之狀態及功用良好，並作出所有合理預防措施，避免引致身體損傷及財物損壞，以及遵從任何公共機構就人身或財物安全制定之所有法定義務、附例或規例。You
confirmed that you have taken and will keep taking all
reasonable care to maintain your insured home and
all Contents and everything used in your insured home in
sound condition and take all reasonable precautions to
prevent Bodily Injury and property damage as well as
complying with all statutory obligations, by-laws or
regulations imposed by any public authority for the safety
of persons or property. |

閣下確認其該投保的居所於過往三年內不涉及任何保險索賠或曾蒙受損失。而且閣下於投保其他保險公司的同類家居保險計劃時，從未被拒絕閤下的投保書、未被取消或未被拒絕續保、或未被要求繳付額外保費或未被要求增加特別條款。You
confirmed that your insured home did not involve any
insurance claims or suffer any loss in the past three
years. Moreover,
when you apply for the same type of home insurance plan
with other insurers, you have never been refused for your
application, have not been canceled or refused to renew
the insurance policy, or have not been required to pay
additional premiums or are not required to add special
conditions. |

閣下明白及接受此類家居保險計劃之保障範圍只包括家居裝修，而家居裝修即指投保人於家居的牆壁、天花、門、地板或窗戶所作出的裝修，而不是物業於新建成後之原有裝修。所以，閣下必須另外購買另一份樓宇結構保險以伸保延障至閣下物業於新建成後以一手樓宇同時交樓之原有裝修項目(如窗戶)，否則該原有裝修將不獲受保。You

plan covers household improvement under home contents
section. Household improvement refers to improvements and
betterments to walls, windows, ceilings, floors or doors,
which then cease to be the property’s original fixtures
as purchased after building construction. Therefore, you
must purchase another Building Insurance Policy to
extend coverage to the original decoration items (such as
windows) of your property which had been completed and
hand overed as frist hand property after building
completion. . |

be issued and NO any printed phyical policy will
be arranged. If you want to get any printed physical
policy, you have to submit your written request and pay an
administration fee at HKD 20 for this. |

responsibilities in relation to the collection, holding,
processing, use and/or transfer of personal data under the

Statement” of LOGOS Wealth.
