Wealth Managment Group Company Limited ("LOGOS
Wealth") is remunerated for its services as
insurance broker by the receipt of commission paid by
insurers. Your agreement to proceed with this insurance
transaction shall constitute your consent for LOGOS Wealth
to receive commission and also to receive your insurance
application and policy related information from insurers. |

complete authority from the Insured Person(s) to disclose
any personal information being requested to assess this
insurance application and make any subsequent request for

confirm that all of
the Insured are Hong Kong citizens or residents holding a
valid HKID card. |

投保人必須為車主及所有司機（包括主要司機及記名司機）年齡界乎23歲至70歲並持有有效正式駕駛執照。The
Proposed Insured must be the car owner and all driver(s)
(included main driver and named driver(s)) must be aged
between 23 to 70 and have valid driving license. |

投保人及所有司機在過去3年內未曾涉及危險駕駛、醉酒駕駛或濫藥駕駛。The
Proposed Insured and all drivers’ driving licenses have
not involved any dangerous driving, drunk driving, drug
driving or penalized with driving license suspension
within the past 3 years. |

在過去5年內，所有司機未曾被暫停或吊銷其執照。所有司機過去2年內扣分不超過9分。All
drivers’ driving licenses have not been suspended or
cancelled within the last 5 years. All
drivers’ driving licenses offence/demerit points under
the Driving-offence Points System should not more than 9
points within the past 2 years. |

所有司機（包括主要司機及記名司機）不曾或正患心臟病、糖尿、癲癇或患有視力或聽覺上的缺陷或身體或精神上的毛病足以影響其駕駛車輛。No
driver has suffered or been suffering from any heart
disease, diabetes, epilepsy or suffered from defective
vision or hearing or form any physical or mental infirmity
which would impair their driving. |

本保險只適合於在香港註冊及行駛的私家車，並只保障在香港特別行政區內發生的意外交通事故。This
insurance is only suitable for motor vehicles registered
and running within HKSAR, and covers motor accident
happened within HKSAR. |

投保的車輛並不是作商業或出租或獲取報酬的用途。The
insured vehicle is not used for commercial purposes or for
hire/reward. |

閣下必須如實填報無索償折扣(NCD)，如有內容失實，保險公司有權由起保日開始取消保單或更改承保條件。You
must fill in the No Claims Discount(NCD) in good faith, in
case of factual misrepresentation, Insurer reserves the
right to cancel the policy or revise the underwriting
terms and conditions from the policy commencement date. |

所有因「已知風險」導致的損失並為不保事項。All
losses caused by the “KNOWN RISK” will NOT
be covered. |

所有經由網上申請，將以電子保單形式繕發，而不會再另行列印實體保單。若閣下需要列印實體保單，閣下必須以書面形式提交申請並先行另繳港幣20元行政費。For
all
online applications, only electronic policy documents will
be issued and NO any printed phyical policy will
be arranged. If you want to get any printed physical
policy, you have to submit your written request and pay an
administration fee at HKD 20 for this. |

閣下安排保險事宜根據個人資料(私隱)條例(香港法例第486章)收集、持有、處理使用和/或轉移個人資料承擔有關責任。
閣下同意進行是項保險交易，即構成
閣下同意豐盛財富之《私隱政策聲明》。LOGOS
Wealth recognizes its
responsibilities in relation to the collection, holding,
processing, use and/or transfer of personal data under the
Personal Data (Privacy) Ordinance (Cap. 486). Your
agreement to proceed with this insurance transaction shall
constitute your consent to the “Privacy Policy
Statement” of LOGOS Wealth.
